STUDENT CONSENT, PAYMENT POLICY & TRAINING AGREEMENT
By enrolling in any course, certification, workshop, private training, or Arizona Board-Approved Program offered by House De Lashes LLC, you acknowledge that you have read, understood, and agreed to the following terms and conditions.
PAYMENT & REFUND POLICY
All deposits, tuition, and payments made to House De Lashes LLC are non-refundable and non-transferable once enrollment is confirmed. Enrollment secures a reserved seat in a limited-capacity training program.
Failure to attend, complete the course, participate, or submit required documentation does not qualify for a refund, transfer, or credit. Payment plans, if offered, must be completed according to agreed terms.
House De Lashes LLC reserves the right to deny participation, certification, or access to training materials if payment obligations are not met.
In certain circumstances, House De Lashes LLC may, at its sole discretion, offer rescheduling or reasonable accommodations when appropriate. Refunds will not be issued.
TRAINING CONFIDENTIALITY & INTELLECTUAL PROPERTY
All training materials, manuals, videos, techniques, educational content, branding, pricing strategies, worksheets, and business methods provided by House De Lashes LLC are proprietary and protected intellectual property.
Students may not copy, record, reproduce, share, teach, distribute, upload, resell, or disclose any training materials or proprietary information without prior written consent from House De Lashes LLC.
Unauthorized use or distribution may result in:
● Removal from the program without refund
● Revocation of certification eligibility
● Denial of future training access
● Legal action where permitted by law
ASSUMPTION OF RISK & LIABILITY WAIVER
Participation in beauty education and hands-on training may involve risks including allergic reactions, irritation, injury, illness, accidents, exposure to adhesives or chemicals, and other unforeseen incidents or unknown.
By participating, you voluntarily assume all risks associated with training and release House De Lashes LLC, its owners, instructors, employees, affiliates, and representatives from liability for injuries, damages, losses, claims, or incidents arising from participation in any training or program to the fullest extent permitted by Arizona law.
House De Lashes LLC is not responsible for:
● Student business success or income
● Client retention or social media growth
● Licensing approval decisions
● Student services performed independently
● Allergic reactions or misuse of products outside training
● Lost, stolen, or damaged personal property
PHOTO & SOCIAL MEDIA RELEASE
By participating in training, you grant House De Lashes LLC permission to photograph, record, and use images, videos, testimonials, student work, and classroom content for business, educational, and promotional purposes including social media, websites, advertising, and marketing materials.
You understand that:
● No compensation will be provided for use of content
● House De Lashes LLC retains full rights to use promotional content
● You waive approval rights related to media usage

STUDENT CONDUCT POLICY
Students are expected to maintain professional, respectful, and safe behavior at all times. House De Lashes LLC reserves the right to dismiss or remove any participant for disruptive, unsafe, inappropriate, or unprofessional conduct without refund.
NO GUARANTEE DISCLAIMER
Participation in any training or certification program does not guarantee employment, licensing approval, clientele growth, social media success, income, or business success. Results depend on individual effort, practice, professionalism, and compliance with applicable laws and regulations.
ARIZONA STATE BOARD ACKNOWLEDGMENT (Applicable to Arizona Board-Approved Programs)
House De Lashes LLC may provide general guidance regarding Arizona State Board registration requirements; however, students remain fully responsible for submitting accurate documentation and meeting all state licensing requirements.
Approval and licensing decisions are determined solely by the Arizona State Board of Cosmetology.
